MEDICAL SOCIAL WORKER

About the Role:

Do you love helping people and making a change in their lives? We are looking for a compassionate Medical Social Worker to join our hospital. In this role, you will assist our patients in using the healthcare system and facilities with ease, provide emotional support to them and their families, and help them receive community resources.

Key Responsibilities:

• Counsel patients on their conditions and medical procedures and be available to them for emotional support.
• Ensure that patients receive community resources like support groups, financial assistance, home health services etc. without delays.
• Prepare discharge plans for the patients and ensure that they receive ongoing support even after getting discharged.
• Liaise with other healthcare professionals such as case managers, physicians and nurses to provide uninterrupted and comprehensive healthcare to the patients.
• Stay current with the new practices in healthcare and medical social work.
• Document patients' treatment history, social care reports and details about other services being offered accurately and confidentially.

Requirements:

• Master's degree in Social Work (MSW).
• Should have excellent communication skills and strong interpersonal skills.
• Must be willing to work in a team.
• Committed to providing high-quality patient care.
• A fair knowledge of government-funded patient care programs, community resources and healthcare systems.
